Confronting Incivility in Nursing

Nurses encounter difficult patients all the time; occasionally, family members chime in and offer their hostility as well. But as part of a team of nurses caring for patients in an already stressful situation, nurses don’t expect to encounter incivility in their colleagues. Aren’t they supposed to have your back? Unfortunately incivility is no longer limited to patients and family members. When viewed on a colleague to colleague basis, incivility is on the increase.

Dr. Cynthia Clark and Sara Ahten, both registered nurses, recently studied this growing trend and found that serious issues such as physical violence can result from incivility as minor as eye-rolling. (more…)

“Too Black”: Georgia Company Allegedly Prevented Black Nurses From Caring For White Patients

ABC reports that a nursing-service company is being sued by four women who allege that civil right violations occurred.

“Accord employs a policy of illegal discrimination in the hiring and placement of its home healthcare employees for the express purpose of accommodating the illegal preferences of its clients,” the lawsuit alleges.

Accord has denied the allegations.

The lawsuit alleges that Accord “routinely declined to place nurses and nurse aides they described as ‘too black’ or ‘too foreign’ or ‘too old.’”

Tracee Goodman, one of the plaintiffs, worked in human resources at Accord for more than two years.  Administrators routinely asked her about the race and age of applicants, giving preference to white applicants, she told ABC News.

Insomnia At Work

Insomnia costs the average American employee 11.3 days a year, or $2,280, in lost productivity. A recent report from the American Academy of Sleep Medicine says that insomnia is affecting 23 percent of U.S. workers and creates a national cost of $63.2 billion for the sleeping disorder. “We were shocked by the enormous impact insomnia has on the average person’s life,” said study author Ronald Kessler, a psychiatric epidemiologist at Harvard Medical School. He also adds that main impact is not that U.S. workers are staying home due to sleeplessness; they still go to work, but are far less productive. The study results were from a national sampling of 7,428 employees. Insomnia was more prevalent among working women than males and also significantly lower among workers aged 65 and older.

Americans are accomplishing less at work because they are tired and, in an information-based economy, it is difficult to imagine a condition that has a more profound effect on productivity. How does insomnia affect nurses in particular? (more…)

How to Interview for a Nurse Job

The nursing interview is the most critical part of the hiring process for a nurse job since it generally determines whether or not you are offered the position. Interview preparation involves both “technical” preparation (rehearsing interview questions, researching facilities, etc.) as well as mental preparation (envisioning success, learning to handle rejection, etc.). Most job seekers focus solely on the technical side of interviewing but are at a disadvantage by doing so since mental preparedness is equally important in terms of a successful outcome.

The In-Person Interview for a Nurse Job

There are four main components to keep in mind during an interview for a nurse job:

  1. Build Rapport: Both you and the interviewer will be looking for ways to establish areas of commonality and build rapport. For this reason, most interviews start off with simple conversation that also serves to settle nerves on each end.

Accelerated Programs Offer a Fast Track to Nursing

Fostering Effective Communication in Nursing

Effective and clear communication is critical during the countless interactions that occur between health care professionals on a daily basis. The staff should know how to communicate effectively with one another so that appropriate information is shared in a timely manner. When effective communication does not occur, the patient’s care is compromised.

The Joint Commission, which accredits and certifies more than 19,000 health care organizations and programs in the U.S., states that communication problems have long been cited as a root cause in the majority of the cases studied. The Joint Commission’s Sentinel Event Database reports that hierarchy differences, conflicting roles and power struggles can all lead to communication breakdowns which ultimately compromise patient safety and the quality of care.

Communication, at its most basic level, is the exchange of information between people, groups or entities. (more…)

What is a Nurse Practitioner?

A nurse practitioner is a nurse who has completed an advanced level of training in diagnosis and disease management. Nurse practitioners work closely with a patient’s regular physician, and can even serve as a patient’s primary health care provider. Nurse practitioners can write prescriptions and are also often solely responsible for first line diagnoses. Nurse practitioners can order diagnostic tests, arrange treatment plans and provide counseling. Additionally, nurse practitioners focus on individualized patient care. Rather than merely treating the illness, nurse practitioners can also provide education and analysis of a patient’s home life in order to prevent further health issues. Nurse practitioners can practice almost anywhere, in all fifty states, in places varying from hospitals to private clinics to school nursing staffs. Many nursing homes have a nurse practitioner on staff, and nurse practitioners can even practice independently. Most nurse practitioners also have a specialty that they focus on in their training, for example, neonatal, pediatrics, or occupational health.

How can you become a Nurse Practitioner?

In order to become a nurse practitioner, you must first have a BS in nursing, and preferably be a registered nurse, although there are some training programs which take those who have the BS but are not registered. Most nurses work for about two years before beginning their training as nurse practitioners. After working as a registered nurse for several years, you can apply for a Masters of Science in Nursing, which also generally lasts about two academic years. While studying, most nurse practitioner candidates also choose an area of specialization, although some choose to remain general nurse practitioners. In addition to the Master’s degree, nurse practitioners must also be licensed in the state in which they want to practice. Each state has different requirements, so you should check your local nursing organizations for more information about practicing in your state.

Nurse Practitioner Jobs

While being a nurse practitioner is extremely rewarding, because of the nursing shortage, some are finding it difficult to get the training they need for certification. There is a shortage of nursing teachers, which limits the number of candidates accepted into training programs. However, once accepted, there are many different job opportunities for nurse practitioners. Because of the varied nature of the job and the degree, nurse practitioners have a lot of options when it comes to choosing how and where to practice. There are numerous web sites advertising nurse practitioner jobs, and it is always worth a try to look at your state board of nursing and local nurse practitioner association. When looking for a nurse practitioner job, be sure to check out the required credentials, as they do vary from state to state. It is also good to search for a nurse practitioner job by your specialty, as this will really narrow down your search, and help you find a job more quickly. Be sure to remember to check local nursing forums as well for the inside information on the job you’re considering.

Five Rights of Delegation

Within the realm of nursing there are Five Rights of Delegation which allow licensed nurses to delegate tasks to other nurses or to unlicensed assistive personnel as long as this criteria is met and the patient’s safety and well-being is not jeopardized.

The Five Rights of Delegation are:

  • Right Task
  • Right Circumstances
  • Right Person
  • Right Directions/Communication
  • Right Supervision/Evaluation

The Nurse Practice Act (NPA) allows for licensed nurses, namely RNs, to delegate tasks to LPNs, CNAs, and UAPs as long as certain criteria is met. First of all the nurse delegating the task must realize that s/he remains responsible for the task and the outcomes.

The Right Task
The RN must determine that the task is one that can be safely delegated in the first place. This includes such factors such as possessing the skills and knowledge of how to perform the task and how to evaluate the success or failure of the task. The tasks must have a predictable outcome, a minimal potential risk, and a standard, unchanging procedure.

The task must fall within the scope of practice and job description of the person to whom it is being delegated and they must have demonstrated competency in performing said task. The organizational policies and procedures of the facility must not prohibit the delegated person from performing said task.

Typically tasks which can be delegated frequently recur in the day-to-day care of patients on the unit. The tasks are not complex, and do not require critical thinking or application of the nursing process.

The Right Circumstances
The patient must be stable and the outcome predictable. The decision to delegate must be based on the nursing process and a careful analysis of the patient’s needs and circumstances prior to delegating. The person to whom the task is delegated must perform the task within their own scope of practice and job description. They are assisting the licensed nurse with the task, not replacing the nurse. If at anytime the patient’s condition changes, the nurse must reassess the situation and may rescind the delegation.

The Right Person
The licensed nurse delegating the task must ensure that the delegatee possesses and has demonstrated the knowledge base and appropriate skills and resources to perform the task and provide adequate supervision and evaluation to ensure the patient’s safety and appropriate outcome.

The Right Directions/Communication
The licensed nurse is responsible to ensure that the delegatee has the previously documented appropriate skills and knowledge base. The nurse is also expected to communicate to the delegatee specific instructions for this task. Each instance of delegation is to be specific to the patient, the nurse and the delegatee.

The nurse needs to clearly communicate the specifics and expectations of the task such as any data to be collected, the method for collecting it and the time frame for reporting results to the nurse. Any possible complications need to be communicated along with time frames for reporting them to the nurse. Further all outcomes must be reported to the nurse and the time frame needs to be pre-defined. The delegatee must agree to accept the delegated responsibility and understands the terms of the assignment.

It must be clear to the delegatee that s/he is not to make decisions or modifications to the care without first consulting with the nurse. This would be overstepping their scope of practice.

The Right Supervision/Evaluation
The nurse remains responsible for the tasks and the outcomes. The nurse is responsible to follow up and receive report on the task and outcomes if the delegatee has not done so. The nurse is responsible to ensure compliance with standards of care, and the policies and procedures of the organization. S/he is to intervene if necessary. After careful evaluation of the outcomes, the nurse also needs to ensure appropriate documentation is provided.

After the fact, the nurse needs to evaluate the patient, the outcomes, and make any necessary modifications to the plan of care. The nurse should also provide feedback to the delegatee and thank them for the assistance.

In some states, LPNs can delegate to CNAs and UAPs. However, unlicensed personnel such as CNAs and UAPs may not delegate. Anyone to whom a task has been delegated may not reassign the task to anyone else.

Senator Boxer Introduces Bill to Ease Nursing Shortage

U.S. Senator Barbara Boxer (D-CA) introduced legislation on May 13, 2009, to help ease the nursing shortage. “We cannot guarantee high-quality care to every American without a high-quality workforce of nurses to provide it,” said Boxer. “That is why I am so proud to introduce legislation to help address our nationwide shortage of nurses. By investing in our nurses, we can help improve the quality of care in our nation’s hospitals and save lives of countless patients.”

The legislation known as the National Nursing Reform and Patient Advocacy Act (S1031), would amend the Public Health Services Act to protect the rights of nurses to advocate for patients. It would set minimum registered nurse-to-patient ratios in hospitals and it would invest funds for training new nurses and nurse educators.

The bill (S1031) has been read and sent to the Committee on Health, Education, Lab and Pensions for review and sponsorship. Nurses should contact their legislators to help sponsor this bill and push the legislation through Congress.

This legislation “builds on the success of California’s historic law that set Registered nurse-to-patient ratios.”  When passed, this bill will extend those ratios to all acute and long term care hospitals across the U.S.

The specifics of the bill include provisions to improve work conditions for nurses by:

  • Establishing nurse-to-patient ratios to improve the quality of care and save lives. By doing so it is hoped that more nurses will enter and stay in the workforce.
  • Providing whistleblower protections for nurses to advocate for patients and report violations of minimal standards of care.
  • Creating a standard of care regarding lifting patients to help protect nurses from on-the-job injuries as well as promote better quality of care.

The bill also addresses funding for workforce development by creating a Registered Nurse Workforce Initiative within the Health Resources and Services Administration that would invest in the education of both nurses and nurse educators by providing grants for:

  • Nursing nursing educational assistance and living stipends for nursing students who agree to work for safety-net health care providers for a minimum of 3 years.
  • Graduate education assistance for RNs who commit to work as nurse educators at an accredited nursing school for a minimum of 5 years.
  • Training and mentorship demonstration projects.

As nurses, we know better than anyone that we are the backbone of the health care system and any health care reform must include provisions for meeting the challenges of the critical nursing shortage by improving working conditions for nurses as well as helping to grow the numbers of nurse educators so that more qualified students can enroll in nursing programs each year and become nurses.
